Up to eight communities receive HCP training each year. Communities begin the process by contacting Dan Frank, program manager for community development, 320-632-9255. He may suggest a site visit whereby he will come to your community to present information on the Healthy Communities Partnership program to interested community members.

Communities that decide to pursue participation in the program will then be invited to fill out a formal application. The time from a site visit to actual program participation is often six to twelve months so interested communities are encouraged to start the process as quickly as possible.
